PRP is a small part of your own blood gathered after spinning. The joint may ache or swell briefly afterward. Relief isn't certain, and the procedure costs money. Ask how the blood is prepared and when you can judge the result.

What is bone marrow aspirate concentrate, or BMAC, and how painful is it?

BMAC uses marrow taken at the pelvis, then spun before use at the joint. That creates two areas that may feel sore. Ask how the pelvis will be numbed and whether someone must drive. Get care directions for the small dressing before leaving.

What is a hyaluronic acid injection, and why is the gel used?

Hyaluronic acid is a normal part of joint fluid. This procedure puts a thick gel into the joint to act like that fluid. Some people seek it for knee soreness, though average relief may be small. Ask how long to wait before deciding whether it helped.

How soon can I walk after a PRP procedure?

Here, PRP means the prepared part of your own spun blood. Walking rules depend on the joint and your provider's instructions. Necessary short walks may be allowed before hikes or court games. Call if warmth, swelling, or soreness rises quickly.

A meta-analysis of 6 RCTs (860 patients, 334 receiving BMAC) found overall complication rates of 41.91% for BMAC versus 41.25% for comparator injectables (P=0.85), with knee effusion the commonest BMAC complication at 18.26%. Early and late complication rates did not differ significantly from HA, steroids, PRP, SVF, MSC or saline. The number needed to harm for BMAC relative to other injections was 152.

FDA states verbatim of stem cell products, stromal vascular fraction (adipose-derived cells), umbilical cord blood, Wharton's jelly, amniotic fluid and exosome products: 'None of these products have been approved for the treatment of any orthopedic condition, such as osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain, or shoulder pain.' The only FDA-approved stem cell products in the United States are blood-forming (hematopoietic progenitor) cells derived from umbilical cord blood, approved only for disorders of blood production, and there are currently NO FDA-approved exosome products.

Medicare's national coverage determination states that CMS covers autologous platelet-rich plasma ONLY for patients with chronic non-healing diabetic, pressure and/or venous wounds, and only inside an approved coverage-with-evidence-development clinical study. Every approved study listed under the NCD is a wound-healing study. There is no Medicare national coverage for PRP in osteoarthritis or tendinopathy, which is why these injections are billed to the patient.

A Danish register study of all intra-articular procedures performed on Funen over eight years measured the frequency of septic arthritis within 14 days of a glucocorticoid injection as LOW, under a local protocol using a non-touch sterile technique with patients advised to seek care on suspicion of infection.
